Documentation required for issuing passports to minors.
The presence of minors of any age is necessary in the Consular Office, by appointment and must be accompanied by both or one of the parents
On the day of the appointment, the following documentation must be presented:
• application form for the issuance of a passport for minors duly completed and signed by the TWO PARENTS, accompanied by a photocopy of their identification documents.
Or, alternatively,
• Application for the issuance of a passport for minors signed by only one of the parents accompanied by a separate declaration of Consent of the other parent (if he is an EU citizen or resident in Italy), with the scan of the identification document of the parent not present, from which it is possible to deduce the correspondence of the signature affixed to the act of consent.
– For citizens of non-European countries, the other parent must instead sign the consent personally in the Consular Office or, alternatively, have the signature authenticated at a Police Headquarters if in Italy or a diplomatic-consular representation if abroad in a country other than India.
In the case of separated or divorced parents – who are unable to produce the prescribed consent – it is possible to apply to the Head of the Consular Chancellery for the issuance of the appropriate authorization if the minor children reside in this consular district
• two recent photographs, the same, front and color on a light background (see characteristics);
• identity document: valid or expired passport, or identity card if it is in possession of it;
For those born abroad, it is mandatory to have the transcription of the birth certificate or alternatively the presentation of the documents suitable for transcription for children born in constancy of marriage or born out of wedlock with joint recognition of the parents. For children born to unmarried parents and not jointly recognized by their parents, it will be necessary to have obtained an appointment with the civil status office and provided the necessary documents for transcription before applying for a passport.
WARNING: WHEN ISSUING THE PASSPORT, THE CONSULATE WILL VERIFY THAT THE DATA CONCERNING THE APPLICANT AND HIS FAMILY UNIT ARE UP TO DATE AND THERE HAVE BEEN NO CHANGES IN ADDRESS, MARITAL STATUS (MARRIAGE, DIVORCE), THE BIRTH OF CHILDREN, ETC. .. WHERE NECESSARY, THE CONSULATE WILL REQUEST FURTHER DOCUMENTATION IN ORDER TO REGULARIZE THE REGISTRY POSITION OF THE APPLICANT AND / OR HIS COHABITING FAMILY MEMBERS. INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED (please pay close attention to filling in all the required fields in the form).
