Duly filled and signed REQUEST FORM – DOV<\/a><\/p>\n Data Privacy Form<\/a><\/p>\n Copy of the passport – first and last page<\/p>\n Final degree\/school certificate in original\u00a0 with HRD authentication (SDM STAMP NOT ACCEPTED) and Apostille + 2 copies<\/p>\n For class 12 certificates a DigiLocker copy of the marksheet, with Apostille can be accepted + 2 copies<\/p>\n (Please ensure that NAME on the PASSPORT and NAME on the\u00a0 DEGREE\/TRANSCRIPTS\/MARKSHEETS is MATCHING.
