For extension of validity of driving licenses, it is mandatory to submit a medical certificate issued by the Embassy’s fiduciary physician:

With effect from Monday, February 21, 2022, this consular service will be available exclusively prior appointment through the portal PRENOT@AMI ( https://prenotami.esteri.it ) on which registration is required.
At the appointment, the applicant needs to submit the original driving license, the medical certificate (with photo) and copy of the payment receipt of the consular fees (validity extension = Euro 41 + stamp duty = Euro 16).
